1. Change of Situation Form - Students use this form to record a change regarding change in family size (birth of a child), award amounts (acceptance, increase or decrease) of their financial aid award, enrollment changes (number of credits or terms of enrollment).

1a. Outside Agency Resources Form - Students use this form to notify Student Financial Aid Services of scholarships or other resources that they are receiving from an organization (agency) outside of the UI.

2. Work-Study Preference Sheet - Students who accept a Work Study award can click here to login and select a position.  Students who are unable to select a job through the on-line process cited above, can click below to download and print a paper Work Study Preference sheet.

- Students use this form only if they have been awarded Work-Study and want to request to work in a different Work-Study position.

5. Consortium Agreement - Students use this form only if they are taking classes at another institution, but would like to receive credit for the classes and financial aid from the University of Idaho.  - Read these instructions first.

5a. Concurrent Enrollment  -Some students are concurrently enrolled at North Idaho College (NIC) and the University of Idaho and want to receive financial aid through UI. Those students must go to the Financial Aid Office at NIC and have them complete a "Concurrent Enrollment Form" for each semester of concurrent enrollment. The form will be sent to the UI Student Financial Aid Services office to document the number of credits taken each semester so that financial aid can be awarded appropriately.

6. Short Term Loan ApplicationShort-term loans are designed to bridge an emergency shortfall between expected resources and payments that may fall due before a resource is available.  Students use this form to apply for an Emergency Short Term Loan.

7. Federal Direct PLUS Loan Application - Parents use this form to apply for a Parent Loan for Undergraduate Students (PLUS) loan.  Parents may borrow PLUS loan funds up to a student's budget minus all other financial aid resources.  Credit checks for the academic year begin in late June.  More information on  Federal Direct Parent Loan Program (PLUS).

8. Household Size and Number in College Verification - Dependent Student - Dependent students use this form only if they have been asked to verify their household size or the number of people in their parents' family who are in college.

Instructions for filling out the Consortium Agreement


To receive Financial Aid from the University of Idaho while attending both Idaho and another institution or solely attending another institution, a Consortium Agreement Form must be completed.

See enrollment requirements for scholarships.

Section I - To be completed by the visiting student.  This consortium agreement must have the name and address of the other institution the student will be attending. By completing this section in its entirety, UI will be able to process this agreement in a timely manner. Note that a consortium agreement is good for one term only.

Section II - To be completed by the visiting institution.  After the student completes Section I, the financial aid office at the school the student is visiting must complete Section II.  By completing this section, the visiting institution agrees to provide grade transcripts at the end of the term of classes the student attended.  Receipt of these transcripts for each term will determine academic progression at the University of Idaho.

The visiting school must send the completed form to the Registrar's office at the University of Idaho.  The address this form should be mailed to is:

Section III - to be completed by the Office of Registrar, University of Idaho.  The Registrar's office has final authority as to whether the credits taken at the visiting institution will be accepted toward the student's degree at the University of Idaho.  If these credits are not acceptable, the student will be notified by the UI office of Student Financial Aid Services that these credits cannot be used toward the receipt of financial aid.  Back to Consortium form selection.
